I am considering buying a 2022 Lexus GX but wife is hesitant because it “requires” premium fuel. Would using regular gas for this specific vehicle cause any harm?

Yes, it will cause problem because the manual says premium gas. To be honest, if your wife doesn't like it because it runs on premium gas, consider something like the Toyota Highlander. I've driven and sat in a 2021 GX. In my opinion, there is not much difference compared to another large Toyota SUV. The only difference is pretty much the badge.

Yes, you should use premium. If this is the issue, you can consider the 4Runner (its equivalent in Toyota line up).
