I have a 2012 Lexus ES350 with 280k kilometers
the A/C blows cool air on the passenger side but normal air on the driver side.
I tried the heater and both sides blow hot air just fine. but when got it back to cooling, the same problem returned.
Malfunctioning vent doors. It's going to be an expensive fix.
That would make sense, however, both sides worked fine when I tried the heater on. and when I switched back to cooling, the driver's side vents went back to normal air. they didn't stuck on heating.
As Joe said, you've got a malfunctioning blend door. If you want to repair it, it will definitely not be cheap.

Correct. But what that may mean is it is not stuck entirely closed. It just isn't going to the fully open position needed to let the full amount of cold air in.
Sometimes a low refrigerant charge can cause that kind of symptom since only one side of the evaporator gets cold. Unfortunately in most vehicles the evaporator is not accessible to check this directly. If the ES350 has a variable-displacement compressor (likely) the only way to be sure the refrigerant charge is right is to evacuate the system and refill with the correct amount by weight.

If your refrigerant is good, then I’d focus on the blend doors..
