My car has been hesitant when shifting. I have attached my live scan data from my bluedriver app

the speed was zero for the entire 74 seconds that you recorded this data. That's not particularly useful for troubleshooting a problem than happens while moving (shifting gears).
Anyway, did you check the transmission fluid level? Have you been keeping up on transmission maintenance?

Check ATF condition, read FAQ on replacing it.
