Ford Triton V10 RV trouble codes leading me in circles.

Summary: (Question below) 

I have a 1999 Ford Triton V10 in a class A RV (F53 chassis) that I bought 2 years ago.  THANK YOU FOR ALL OF YOUR HELP WITH KEEPING MY DREAM ALIVE.  I have only ever had good luck with Fords so I swear by them - never has one left me on the roadside. 

Problem:  1)  I am not a mechanic and only found you about 8 months ago. 

               2)  Since purchased, I've noticed a hesitation/sluggishness at highway speeds and outright chugging whenever the RPM's go below 2000.   When I was picking it up, a technician let slip that they had replaced two spark plugs during prep.

- Please note, I found out shortly thereafter that when the gas tank was filled....it has a leak.  I was told, that could cause "a problem with the engine and how it runs".  While waiting for someone to help me with that I tried doing my best to reclaim a neglected engine.

- She always starts and runs without an issue.  No stalling.

Steps taken:  

-regularly replace fuel filer, air filter, oil change, grease drive train and premium gas.  Also replaced leaking rad hose that was dripping on ignition coils for cylinders 9 and 10. 

- One can of Seafoam cleaner in about every fourth tank.

- Bought ODB bluetooth scanner and connected it to my tablet on my dash, so I could see what the motor was doing..

  • HP output has never been above 200...specs say I'm about 200 short.
  • Torque has never been above 345....specs say I'm about 130 short.
  • Mileage has increased from about 36l/100km to 47 l/100 km.
  • O2 sensor swings dramatically from lean to rich.
  • Fuel trims bounce erratically.

This improved performance (and made the hesitation/chugging milder, but chugging remained).

- Just recently, with my last oil change, I located the PCV valve and found that it rattled when I shook it, however, I noticed that the hose connecting it to the intake sucks the sides together when the engine is running...(noticed this yesterday).

I receive all kinds of codes sporadically, from misfires on 9 and 10, to transmissions sensors reading out of range...some I have forgotten because they haven't reappeared.  

I recently ran the gas tank near empty and filled up at a gas station who was near empty as well(...Delivery next few days...). 

Shortly after, my problems returned with a vengeance. I have no more than 98 hp and barely enough torque to break wind. The hesitation is unbearable and there is little power on the highway - chugging is gone though.  Have changed the fuel filter and air filters etc.  

Now, I am regularly seeing transmission error codes but shifting is smooth - before the service engine light comes on and it stays in 3rd.  When it does, I shift manually, and it behaves smoothly as well.

Question:

I just reviewed your video on cleaning catalytic converters after I noticed exhaust gas coming from the manifold side seams on startup (its cool up here in the mornings).  I plan to do it as soon as I get a chance. 

Have you seen wrong error codes from Fords when chasing a problem like a blocked catalytic converter, or should I prepare for some expensive transmission repairs?  

I will take all suggestions seriously, so I thank anyone that does and Scotty for all of his videos...you've saved me more than once!!!

I would clean the MAF sensor and throttle body. Do not use the Seafoam in your truck. Have a mechanic pressur clean the injectors. 
Also, pressure test the fuel line.
