I've got a 2006 Toyota Sienna with 200,000 miles. There are a bunch of gears on the shifter I've never used (4, 3, 2, L). Is it a good idea to use those gears from time to time for the health of the transmission?

That transmission automatically shifts between all those gears as needed. Those selections on the shifter are for manual control if desired.

sure why not. Clear the fluid out of those channels.
