Puttering on a 2004 Toyota Prius, 295k miles

The last 45K miles or so, the engine putters when moving along. No codes.  No difference in performance. No symptoms.

But the engine putters. It sounds old. And it is old. It putters more frequently when the foot is fully on the throttle. The putter is lower in volumes and less frequent when foot is lighter in the throttle.  And it is only apparent when the ICE is engaged.

Imagine you are fluttering your lips a low speed. Not blowing out fully. But slowly.  That’s what it kinda sounds like  

1) Given the age and mileage of the vehicle, what could be the cause of the puttering?

2) And with that cause, what are the potential fixes?

3) Or is this how old engine sound?

well they shouldn't make any AD noises could just need a tune-up realize they have coil-on plugs and spark plugs and all those things can wear out and any good mechanic like myself can analyze the live data with a scan tool and come up with a pretty good idea of what needs to be done
