I have a 2000 Toyota Sienna with 120k miles. No engine codes, but misfires on cold start when I put it in gear. Maybe 5-10 misfires pretty consistently every cold start. Also, it sometimes misfires after I stop at a red light. Once the engine warms up it runs perfect.
Back in September I installed new spark plugs and it ran perfect for a month. Now the cold-start misfires are happening again.
I am planning to look at the plugs, install a new set of denso plug wires, and change the inline fuel filter. Any other ideas?

Have you cleaned the MAF sensor?

I installed a new Denso MAF a year ago, but I will add that to the stuff I do next. I was going to do a throttle body IAC valve cleaning soon anyway.
