My brother has a 2016 toyota yaris with a manual transmission and has 120,000kms (around 75,000 miles). It makes a squealing noise when you start the car, but if you turn the key and leave it in "on" for 5 seconds it doesn't make any sound. We checked the belt and seems fine (no cracks) could it be the tensioner? Or what could it be?. Thank you

A belt can be worn without showing cracks. Take the belt off and start the car. Do not let the engine continue to run, you only want to see if the squeal is still there. This will let you know if the sound is from that belt's driveline.It makes a squealing noise when you start the car, but if you turn the key and leave it in "on" for 5 seconds it doesn't make any sound.
