Hello Scotty,
I have a 2005 toyota highlander (200k miles) and 2005 Lexus RX 330 (118k miles) both has 3.3 v6 engine , both car Transmission struggling to shift from 2nd to 3rd. Fluid is clean and being maintained regulerly. they have rough shift from 2nd to 3rd gear when accellerating to around 30 miles. I noticed this in low RPM.

If it happens regardless of the engine temperature, it is likely die to internal transmission issues. Have a transmission guy to check it for you.
