Have a Camry with 260k. I drive mostly highway miles. While I am driving at 60-70 mph at very low RPM, and requires to reduce the gear to 5th it will make a clunk noise and hesitate changing gears to 6th back to 5th couple times. Usually I have to push the pedal to let it stay in 5th gear and increase the RPM. It started doing this during this last cold and harsh winter.
Any suggestion ? feels like when the engine misfire. I ran a code checker and it is clear.

Is your transmission fluid good?
Given that most of your driving is highway miles, it's not surprising that fifth and sixth gears (particularly sixth gear) is giving you the most trouble. After all, it's the one that is in play the most, given your driving pattern. So . . . I have some questions:
1. Have you ever had the transmission fluid drained and filled? (Scotty, along with other experts, such as Louis Althazan claim, correctly, that flushes hurt more than help).
Despite what Toyota officially says, even Master Toyota Diagnosticians (such as thecarcarenut) recommend drain and fills, every 60K or so.
2. Have you ever had a small amount of the transmission fluid drained out for evaluation? You'd be looking for color, and pieces of metal in the fluid or attached to the drain plug.
3. Are you willing to have the pan dropped, to determine how much metal will be found at the bottom of the pan, attached to magnets?

So, it seems, at least to me, that you'd need to take a look at a small fluid sample, if not what's in the pan, before you decide what to do. If the gears are indeed worn from use (and if the fluid has never been changed), you might just want to live with a de facto five speed. The gas mileage will be a bit worse, but just a bit. Unless the rest of the car is in primo shape (and any such assessment should depend on an independent assessment by a qualified and honest mechanic), it's probably not worth it to put in a remanufactured tranny, given the miles (and perhaps the years, this is a 2012-17 vehicle, apparently). But, get an accurate diagnosis and cost structure first, before making any decisions on the future of the vehicle.

Well it's automatic transmission that's just how they wear out I would personally live with that
