2012 Prius with 168k miles.
Twice in the last couple of months I have experienced an awful rattle and shake come from the engine on my Prius after driving to my destination and then restarting after a short while to come home. The oil level is fine and looks pretty clean. I have seen several others with the same exact issue on COLD starts and suggesting blown head gasket, EGR, clogged fuel injectors ect. Where should I start investigating?

Other Toyotas of that vintage are know to have noise on startup from the VVTI mechanism, and there are TSBs on that.
Clarification: No check engine light at all.
Happened this morning on cold start. Coolant level looks the same and is still clean. Since it is cold out (45) there is steam but i dont think smoke from burning coolant.
