My wife's 2014 Corolla has 140,000 miles and the CVT transmission has never had a drain and fill. Is it safe to perform a drain and fill with this high mileage? Or would it be best to leave it as-is?

What does the fluid look like? Is it clean or dirty? Changing the fluid after this many miles is a gamble. It is supposed to be on its third fluid change by now.
it's a CVT, doc. No clutches.

Yes it's safe. You're confusing it with a conventional automatic.
You should be doing it every 30-40k. CVT fluid gets contaminated quickly.
