We don't see many Hiluxes here in the US. I was directed to your post a couple of days ago and I didn't have time because I had questions.
Looking at the wiring diagram you posted the 12 volts from the EFI relay comes in on the Red wire to pin 3 of the MAF sensor.
Pin 3 is the center wire.
However, your picture of your MAF connector shows the Red wire is on the end.

So, is that a replacement wiring connector pigtail for the MAF sensor?
Here's the wiring diagram you posted above. I added some text.

That diagram didn't include the two wires for the IAT sensor.
The old 2.4 Tacomas we have here used the same MAF sensor as your Hilux so I pulled up that wiring diagram. The way it's wired looks identical to the wiring diagram you posted.

Pin 3 is Power from the EFI relay (EFI relay isn't shown. It's on the previous page of the wiring diagrams and I'll post the link at the end of my reply). Pin 4 is Ground provided by the computer. Pin 5 is the 0 to 5 volt MAF Signal back to the computer.
As far as the other two wires for the IAT sensor. Pin 2 is the reference voltage and Pin 1 is the computer provided Ground.
So like you originally posted, "I need to know which cable is which, the 2 grounds, 5V, 12V and signal"
Grab a multimeter and identify the 12 volt wire, and the 2 Grounds.
Unplug the MAF wiring connector. Turn the Key on. Multimeter on 12 volts, (20 volt range). Attach the black probe to battery negative and probe that center terminal on the MAF connector with the red lead. It should show battery voltage (12 volts + or - a little)
Now put the test light or multimeter on battery positive and Probe terminals 4 and 1. They should be the computer provided Grounds.
If you want to take it a step further you can remove the MAF sensor and watch the resistance change on your multimeter between MAF's IAT sensor terminals 1 & 2 as the sensor heats up. Check it in your house while it's nice and cool with your AC running. Then take it outside into the heat and note the resistance change.

That will confirm that we're right about MAF sensor terminals 1 and 2 being for the IAT thermistor.

1 is ground, 2 is reference voltage but I'm not sure how this reads on a multi tester, 3 is 12V, 4 is ground and 5 is the 5V signal (I'm confused between this and 2)
And is 1 red and green 5 in my connector or green is 1 and red 5?

Hi, I need to know which wire goes where for this MAF sensor (22204-75020) it is not wired currently. I need to know which wire does the 5V, 12V, grounds and signal. I used the wiring in a video about a corolla because the sensor lookes the same and ended up frying the MAF I had, now I ordered a new one and don't want to mess it up.

Where is the image of the wiring diagram? It's not coming up.
The wiring diagram is NOT coming up. What is the tag, name, number???? All I'm getting is a picture of the plug.
