Hey, I have an old 1995 Mercedes C280 with a 2.8 inline 6 and a speed automatic, and it's about to reach 230k miles, you can feel the gear shifts, and sometimes I can hear a groaning or grinding noise in the back and not upfront where the transmission is. could this be a sign I need to change the fluid and filter or be caused by a known vacuum leak, or do I just drive it till it dies? Also, would a 2005 E320 be a good replacement?

Have you serviced the transmission before? If not, I would not touch it and keep driving till the tranny gives up.

No.
Weird shifts: Can be caused by a vacuum leak. Hard shifts/clunka-clunka can also be caused by the hardy-couplings in the drive shaft or worn rear bushings. Grinding noise in the back: Check your rear wheel bearings and diff oil.
Noisy transmission can be caused by old oil/low level.
With that age and mileage, most likely there is a combo of several issues.
