I have a 10 year old car. In looking at the undercarriage, the only rust is surface rust on the exhaust. Does it help the rust on the exhaust from getting worse if you remove the surface rust? Was planning on sanding it off and maybe using vinegar as well.

An steel exhaust lasting 10 years sounds as a miracle to me...
On my cars they rusted inside-out and broke due to vibrations in much shorter time. 😥
Just drive your car. I have a 1999 Ranger in the middle of the rust belt and it's fine. Everything but the final segment of tailpipe is original after almost 300k miles. Ford used cheap steel for the actual tailpipe, and it partly rusted off. It was still attached to the vehicle on the other end and it really annoyed me, so I replaced it. Everything else about it is stock after 24 years.
