Vibration at idle - engine crooked?

2009 Pontiac Vibe GT, 194k, Toyota 2.4l engine, 5 speed auto trans.

I have an annoying vibration between about 800-1200 rpm. There is also a jerk or jolt when I start the car. It doesn't change intensity based on load. In drive, neutral, or park, it's always there in that rpm range. I've had all the mounts replaced, but the top passenger side mount will fail within a matter of minutes. It's a hydraulic mount, but the others are just rubber and steel and are in good condition. Replacing this mount fixes everything at first. I won't feel the car start, and there is no vibration under any condition. Then, the vibration gradually comes back in about 5-10 minutes of driving. My mechanic said my engine is misaligned by 2 degrees, which is stretching the mount and causing it to fail. I had the engine rebuilt a few months ago and did not experience this particular vibration before that. I did get vibration prior to the rebuild due to worn mounts, but it would go away in park or neutral as expected. You can see the engine vibrating under the hood, but not before the rebuild. I've never wrecked the car or hit anything worse than potholes. Any ideas on this? Thanks in advance,

Well if it isn't the transmission starting to go out and the motor is misaligned that would mean the motor mounts I probably not the correct size for the vehicle if they are all bolted in and not cracked many aftermarket mounts do not fit right so the motors do not sit correctly that's why I always use only OEM motor mounts from the manufacturer

All the mounts are OEM.

The mounts are all Toyota mounts purchased from a dealership. They are all pretty much new too. I can't imagine how, but could they have been installed incorrectly? Is it possible that something else is causing enough vibration to make that one mount give out so quickly?
