I have a 2001 dodge ram v6 truck 156000 miles on it with an automatic transmission. Half of the time when I am driving, i have to let my foot off of the accelerator to make the truck shift, or the rpms will stay really high. Is that bad news for me and a transmission repair, or is it just old and it doesn't mean my transmission is going out?

I have bad news for you..
Try changing the fluid & filter, but if it persists, live with it till it goes out completely & then get another vehicle.
