Hi, I'm looking at a 2004 sienna, fully loaded fwd. In good condition (per ad) with 215km (133k miles). Listed for $5000 CAD ($4000 USD). I've been waiting for something like this, but with lower km's. Is this a good deal? How many more km's should I expect to be able to get on it?
What kind of issues do i need to be aware of or ask about for this model and year?

According to what Kerem posted on Carkiller, the 2004 Toyota Siennas have a problem with the dashes being cracked. Check with the mechanic for any problems and check the titles too. If it's a good condition, take it right away since the price is very low. You can get 300,000 miles or more with the 2004 Toyota Siennas.
The Toyota Sienna is a good minivan. The thing to have in the back of your mind, though not a deal breaker, is that 2004 was the first year of the generation. Kinks weren't worked out yet. Do would want to make sure the dash isn't cracked or melting like the countless Toyota Lexus vehicles of the time, and also the timing belt. Internet has conflicting info on whether the engines are interference non interfereced but if it has a timing belt you would want to change it. I'd pay a bit less than that.
Definitely have a mechanic check it out. Keep in mind though that it is 17 years old so you might need to put some money into it if you have to replace stuff under the hood because of age.
I took a closer look at the pictures and the dash does seem to have crack in it. Is this a structural issue or simply cosmetic?

I looked at some images and the complaints and it seems there is a risk of the airbag throwing the dashboard into your face... Its better than the Lexus dashboards which just melt away but I'd check the complaints above. I'd definitely use it to bargain hard and if you get to like 3000 USD who cars the airbags usually don't work anyway at that age.
