Just bought a 2007 Honda Pilot with 123,000km on it. It seems like a very reliable vehicle but I have been reading about issues with the window regulators where they fail and drop the window into the door without any warning.
My question is: What causes this failure (corrosion, lack of lubrication, poor design, etc) and is there preventative maintenance that can stop this from happening?
Also if there is a failure is there a more reliable aftermarket replacement?

There is no preventative measure you can take as this is a design or material flaw.
In your shoes I'd try an aftermarket regulator because buying another OEM will be just replacing a poorly designed/made regulator with another poorly designed/made regulator. I'm not saying that an aftermarket will be any better, but the OEM sure isn't any good.
Here's where the problem is with those.

They're cable driven. A plastic piece on the regulator carriage that connects the cable to the carriage breaks. When the cable disconnects from the carriage the window goes crashing down.
Here's what the plastic part that holds the cable in on the carriage looks like when it breaks.

Owners tried suing Honda over this but they weren't successful.
Any kind of preventive measure will probably require disassembling the door.
Might as well just wait for it to fail (if ever) and then replace it.
