I purchased a 2011 Honda Pilot with 209k miles. It had a Code P420. I did the lacquer thinner trick and it didn’t resolve. Ran a Carfax report and it states the Cat was replaced 12/2020 (155k mi), 04/2022 (171k mi), and 11/2022 (180k mi) by the same repair shop. Thoughts on what can cause the code to continue? Could it be a bad O2 sensor?

More often than not, P0420 is indicative of a cat needing to be replaced. If it has been replaced several times, it might be a good idea to look upstream or downstream from the cat, like the O2 sensors, or even further up with spark, fuel, and air…. But do a diagnosis first before throwing parts at it.
