CRV shaking at certain throttle setting

I have a 228000 mile 2007 Honda CR-V (automatic 5-speed) that shakes when cruising at low throttle and rpm. Whenever I'm driving through a 30 mile speed limit street, when I'm cruising at 1400 to 2000 rpm with the throttle 18 to 20% open, the car starts shaking. When I remove my foot completely off the pedal the shaking stops, and when I accelerate and open the throttle to 25% or above the shaking also stops and the car accelerates. It usually happens when I'm cruising is 2nd or 3rd gear, and when I open the throttle and accelerate, the rpm would rise and then the car would shift normally. The shaking is in the engine/drivetrain, not the suspension. Is this a transmission issue? Gas issue? Throttle issue? Thanks.
