I’ve had a 2003 ML 500 with 186,000 miles for 20 years now. It has the legendary M113 V8 engine. I’ve had no issues with it, but am wondering if I should change the timing chain and tensioners? It has the original one on, but not sure if there are engines out there that never need to have a timing chain replaced?

You only touch the timing chain when the engine is seriously misfiring. If you want to, scan your vehicle for any codes and see if any one of your cylinders is misfiring. If there are none, leave it alone.
