[Solved] 2009 Prius 244000km Death triangle no codes

I bought a 2009 Prius with 240 000kms in it and after about 3 days there came a big triangle warning on dash with exclamation mark. I used a OBD2 scanner but no error codes.

Then I went onto check battery (12V)

without engine ON volt is about 11.5 with engine ON it is 13.9-14.1 V

Also I realised after car sits for about 2 hours no Big triangle on dash too.

Does this mean my 12V battery is bad? or brake actuator failure (as a friend of mine suggested)

Well it could mean all kinds of things. But the voltage is way too low on that battery when it's not running. Realize it gets charged by the generator/ motor when the car is running then the voltage is correct. So start by replacing that little battery

@scottykilmer I also noticed that triangle warning do come back after the car sits on hot climate (around 35 C). Any connection between the hot climate and 12V battery draining (ultimately popping warning lights?)

12V battery draining

Scotty has already mentioned a weak 12V battery can cause this. And that many things can cause this so you need to rule out the 12V battery first, test it - make sure it passes a load test.

I scanned the car using a OBD2 + Dr prius app: P3000P0A84 battery running warm-These are my issues..so basically I may have wanted money on a new 12V battery already

P3000 & P0A84 indicate an issue with hybrid battery's cooling system. (something within that system isn't functioning properly) These links should be helpful: https://www.autocodes.com/p0a84_toyota.html#descp1   https://priuschat.com/threads/2005-prius-codes-p3000-123-and-p0a84.217561/#post-3052385

After 200kms drive the warning is back-yesterday afternoon was bit hot and I switch on the Auto A/C when I get into the car. After about 2 min , the warning was back

Did you check that the cooling system for your hybrid battery is working OK (vents not clogged, cooling fan working, etc.), seeing as how @hixster pointed out that's the kind of problem your codes indicate?

Although the following video is about clogs due to pet hair it shows how the hybrid battery cooling system works:

You can also find a repair manual with details on the hybrid drive system (including cooling) at the link below:

Turning the AC doesn't cool the NiMH battery, if that was your intent. The NiMH battery has its on dedicated cooling system as has been noted here several times. Fix you Hybrid battery's cooling system or you will ruin it.
