Engine dosen't warm up

Have a 1999 dodge ram 2500 with 5.9 engine 130,000 miles. Engine will not get above 150 degrees. The heater hoses get warm and will get warm in cab until on the highway, then cools off. I changed the thermostat and nothing, I bought another thermostat and tested it and still nothing. replaced water pump, flushed the radiator and new cap.  The clutch fan seems ok. Any ideas? Thanks

Did you bleed the cooling system? You need to park the truck on an elevated surface, engine up, and start it. Slowly pour antifreeze into the vehicle until it approximately reaches  the neck of the radiator. Turn the heater to max heat and set it to full blast.

Do you mean on the gauge?

tried bleeding it and the gauge shows about 150. radiadtor is cold.

Expose the thermostat to boiling water. The thermostat should open quickly. Go with an OEM thermostat, not a cheap aftermarket one.

i tried that with another new thermostat. it worked fine in a pot. same problem in the truck. runs like no thermostat in it

I agree with @justin-shepherd , everything points to a bad thermostat(s).

with infra red thermometer heater hoses over 200

and where would the 200 degree water come from?

Engine has to be at least 200.

had the thermostat in a pot

yup I saw that . Clearly the engine IS warming up. The 150 you saw just isn't accurate.

Hmm something's amiss. I believe @justin-shepherd wanted you to make sure didn't have air trapped in the cooling system (heater core or block) by you 'burping' the cooling system. That requires parking 'uphill' (the steeper the better), setting the heater temp to max, removing the radiator cap, crank truck and allow it to warm up. Because the front of truck and radiator are higher than the heater core and block, any trapped air should escape through the radiator neck. Normally you will be able to feel the hot coolant flowing through the top radiator hose as the engine warms up.    I'll assume you've done all this for now.

Are you sure the temperature gauge is working correctly? (An easy way to check this is by using an infrared temperature gun) The reason I ask is because to my knowledge only a thermostat hung open would create this condition.

had it parked on a ramp when i filled it. truck gets heat. heater hoses are hot and top radiator hose gets warm. with infra red thermometer heater hoses over 200 ,everything else around 90

radiadtor is cold.

it's plugged then. Radiator should be around the same temp as heater.
