Surging idle in drive not in nuetral

2000 K2500 Chevy Old Body style. 5.7L engine 165000 miles. Engine once warmed up surges/lugs while idling( in dirve). Switch over to neutral and it goes away. seems to only do it when warmed up and randomly The idle lumps. So it will drop in RPM's then go higher. Anywhere from 400-800 RPM then over and over sometimes getting worse until i shift to neutral or slightly step in the gas with my foot on the brake. Any Ideas?

Clean the MAF sensor. 


should have said, I had just done that last weekend. Thank you for the idea!


Do you have access to live data? Check the MAF reading. I want you to make sure it works fine.

And get a new air filter.


did that when i cleaned the MAF last weekend. THank you!

I would next look at the trans mission fluid and filter since it seems to load up the engine when put in drive.
