2008 Suburban 5.3 engine, 197k miles, tranny filter service was done at 100k. Tranny runs perfect... Checking fluid it is still red and feels and smells normal, should I bother changing filter again or leave well enough alone?

yeah do it now, then at next oil change then again every 30k miles

Drain the fluid, drop the pan, replace the filter, reinstall the pan, refill with proper fluid. That long without maintenance is asking for trouble. What does the owner's manual say?

At that mileage I’d leave it alone and drive it til it dies.
