What's the deal with gas flying out of the fill hole at the gas station?

2001 Toyota Seinna CE. 250000 miles.

When all the computer checks finally completed on my crummy new beater (the evap checks took thousands of miles for some reason), I ended up with a P0442 code. I also noticed that at a few stations, when I was getting gas and I used the hands-free lock, when the gas was done filling, some gas would come flying out of the tank hole. It would click and then spray forth. I would normally let it fill from 1/4th tank, if that matters. Is that a P0442 code sort of behavior? Or, perhaps I should just ask: "Why would gas spew like that?".

Next time, don't lock the fill lever in the full down position.  The fill tube spring loaded door that you put the nozzle through is probably failing and the fill pressure is too high with the lever in the full down position.
