I was looking at a Carfax on a certified 2020 pre-owned Toyota Tundra 5.7 V-8 that I was considering to purchase. It appears, (according to the carfax) the first oil was completed at 21 thousand miles. Should I be concerned with this carfax data? This oil change periodicity has my teeth chattering just thinking about the possibly long term engine damage?

Well, let's remember one thing: if the oil change was done by the individual, it will not show. And certain automotive shops do not show either.
The quick "pass on it" responses here back up my point I've always stated about the guys here on the forum who like to DIY their own maintenance. Realize that if you do that, CarFax will not show it. So to the next buyer, maintenance has never been done and you'll have a lot of people pass on buying your vehicle based just on that.
If it makes you uncomfortable, pass on it. I would just because I'm not much of a dice roller.
Great point! Which is why it’s very important to keep receipts or any kind of documented proof/evidence of any/all repair or maintenance, performed on the vehicle.

I have noticed on some vehicles (I am taking about Lexus here) that the services performed don’t always appear in Carfax, but when I enter the VIN on Lexus’ website it lists all the services done (and at which mileage). You have to set up an account with Lexus (for free) but let’s you get more information like that over Carfax. I am not sure if Toyota has that as well - it’s worth looking into it.
Otherwise, if there is no recorded history of oil changes (and the seller/dealer does not know), then I would move on.
It should be still under warranty. Find out about the dealer who sold the car and talk to them it you want to find out the truth.
