I have a 2004 Lexus is300 with 100,718 miles it’s an automatic. My steering wheel shakes like mad at around 50-60 mph I’ve had my tires balanced and my suspension checked by a mechanic who said everything was fine but I still have the same problem any idea what it could be?

Are you able to rotate the tires to see if the vibration lessens or moves if you move the front tires to the back?
It could be bad alignment or a bent wheel (sometimes bent rims can still balance correctly). See if they had to put a huge amount of weights on the wheel when the balanced it. Could be a sign of a problem with the rim.
I’ll rotate them front to back and see if that helps
Try swapping the wheels front to back and see if it continues. I know you have them balanced but that sounds just like what happens when wheels are out of balance, but also could be an issue with the tires themselves.
At shake speed pull it out of overdrive into drive, still shaking? If so, find another tire/suspension mechanic, rebalance and rotate as mentioned above. If shaking stops, then check motor and trans mounts along with driveshaft coupler/dampener.
